THIRD-PARTY ADMINISTRATOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.
ELIGIBILITY: The 2027 U.S. Wellness Activities window is open to Bank of America employees (“Eligible Employee”) and their eligible spouse, partner or other adult dependent (“Eligible Dependent”) who are enrolled in a wellness-eligible medical plan sponsored by Bank of America and administered by Anthem, Aetna, UnitedHealthcare, or Kaiser 2. Eligible Employees and their Eligible Dependents are referred to herein as “you” or “your”.
AVAILABILITY PERIOD: If you are enrolled in a wellness-eligible medical plan through Annual Benefits Enrollment with coverage beginning January 1, 2027 (if your coverage is ongoing from the previous year), your 2027 U.S. Wellness Activities window begins at 8:00 am October 1, 2026, and ends at 11:59 pm on February 28, 2027. If you are newly eligible and enroll in medical coverage effective on or after January 1, 2027, you can start the 2027 U.S. Wellness Activities on the day your medical coverage takes effect, and you generally have two months after your medical coverage takes effect to complete the wellness activities. To find your wellness activity deadline, navigate to 2027 Wellness Programs section on the Home page to see the number of days remaining on the Health Assessment and Screening and Annual Physical with PCP activity cards.
REDEMPTION: To keep a wellness credit of up to $500 (or up to $1,000 if an eligible dependent participates, too) toward your annual medical plan premium, you must complete and submit all three 2027 U.S. Wellness Activities on Well.co by your deadline. This wellness credit is comprised of:
- $250 (for Eligible Employee and Eligible Dependent) if you complete the health risk assessment and health screening activities by your deadline; and
- $250* (for Eligible Employee and Eligible Dependent) if you attest to having a primary care provider (PCP) and having had an annual physical with a PCP in the past 12 months activity by your deadline.
*Note: While you can complete the 2027 U.S. Wellness Activities in any order, to keep the $250 credit for attesting to having a primary care provider (PCP) and having had an annual physical with your PCP in the past 12 months, you must also complete and submit the health risk assessment and health screening by your deadline.

Wellness Activities Waiver
The wellness activities are designed to promote overall health and prevent disease. But if you’re pregnant or it’s medically inadvisable or unreasonably difficult for you to participate in the wellness activities based on a medical condition, you may submit a Healthcare Provider Medical Waiver form signed by your health care provider on or before the wellness activities deadline. This signed form must be submitted by your wellness activities deadline and may be submitted in place of completing one or all of the wellness activities noted above. The Healthcare Provider Medical Waiver form may be accessed online, by phone or by email:
Online: Log into My Wellness (mywellnessbofa.com) > Help Page > U.S. Wellness Activities
Follow the Submission Instructions on the form.
If you have questions about the waiver process, please contact Well Member Services at bofa.mywellness@support.well.co or by calling 844.939.5100. Representatives are available Monday through Friday from 8 am to 9 pm ET.
If you’re enrolled in a wellness-eligible medical plan and re-enroll in a wellness-eligible plan as part of Annual Benefits Enrollment for coverage that starts on Jan. 1 and submitted a medical waiver, it must be dated between March 1 of the year you enrolled and the end of February of the new plan year, and it must have been received by your wellness activities deadline. If you’re enrolling in the medical plan during the year and are submitting a medical waiver, it must be dated within 12 months before your medical coverage effective date and must be received by your wellness activities deadline.
Your health care provider will indicate the activity the waiver covers. If your waiver doesn’t cover all of the required wellness activities, you’ll still need to complete the activity that isn’t covered by the waiver. The results of the non-waived activity must be submitted by your wellness activities deadline to maintain the wellness credit.
