“Bank of America Annual Wellness Activities Challenge” Rules

TERMS & CONDITIONS

NO PURCHASE NECESSARY. Void where prohibited. All federal, state, and local laws and regulations apply.

SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to: (i) Bank of America employees who are enrolled in a health plan offered by Bank of America and administered by Anthem, Aetna, or UnitedHealthcare (the “Health Plan”), (ii) the spouse, domestic partner or other adult dependents (OADs) of such employees; provided that the spouse, domestic partner  or OAD is also enrolled in the Health Plan, (collectively, “Eligible Users”), (iii) Bank of America employees who are enrolled in a health plan offered by Bank of America and administered by Kaiser, and (iv) the spouse, domestic partner or OADs of such employees; provided that the spouse, domestic partner or OAD is also enrolled in the Health Plan, (collectively, “Restricted Users”).

OFFER PERIOD: The “Bank of America Wellness Activities Challenge” (the “Offer”) is available beginning 8:00 am October 1, 2025 and ends at 11:59 pm on February 28, 2026.

OFFER REDEMPTION: Bank of America will award rewards or premium discounts, as appropriate, to Eligible and Restricted Users who complete the participating actions of the Annual Wellness Activities Challenge – Level 1 and Annual Wellness Activities Challenge – Level 2 during the Offer Period as defined below.  Users may complete the Annual Wellness Activities in any order (i.e. as between Level 1 and Level 2).

Upon completion of the Wellness Activities Challenge – Level 1, Eligible and Restricted Users will receive a 2026 wellness credit of $250.

Upon completion of the Wellness Activities Challenge – Level 2, Eligible and Restricted Users Users will receive a 2026 wellness credit of an additional $250 (for a total of a $500 wellness credit).

TO PARTICIPATE:

Eligible and Restricted Users must first create an account with Well.
On mobile: Download the “Well Digital” app from the App Store or Google Play. Enter your Bank of America email address, date of birth, and mobile phone number to activate your account.
On the web: Visit app.well.co  Enter your Bank of America email address, date of birth, and mobile phone number.

To Complete the Wellness Activities – Level 1 of the Annual Wellness Activities Challenge
(1) Complete the health risk assessment on Well.
(2) Complete a biometrics health screening.*

To Complete the Wellness Activities – Level 2 of the Annual Wellness Activities Challenge:
(1) Complete a PCP visit / primary care attestation.

*Note: this step is not required in order for older adult dependents of Restricted Users who are enrolled in a health plan offered by Bank of America and administered by Kaiser to complete Level 1.

Please allow reasonable and appropriate time periods for processing of rewards and/or the application of wellness credits. Bank of America and/or its agent will be the sole distributor of wellness credits, via payroll distribution. Please refer questions related to such distributions to the Global Human Resources Service Center  at (800) 556-6044. 

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

 

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
