“Workplace Wellness” Offer

TERMS & CONDITIONS

NO PURCHASE NECESSARY.  Void where prohibited.  All federal, state, and local laws and regulations apply.  

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; and (ii) a registered member of the Well application.

OFFER PERIOD:  The “Workplace Wellness” Offer (the “Offer”) is available beginning at 9:00 a.m. ET Monday, June 3, 2024 through Friday, July 5, 2024 11:59 p.m. ET.

OFFER REDEMPTION: Sponsor will award 100 bonus points each week during the Offer Period to each Eligible User who completes a weekly Workplace Wellness chat containing the weekly “code word” Maximum 400 bonus points.

TO PARTICIPATE: Chat the prompt word or phrase specified in the emails with the following subject lines within 7 days of receiving each email: “(Your First Name), earn an extra 400 points this month”; “⏰ It’s time for a wellness break, (Your First Name)”; “Your Workplace Wellness points are waiting”; and “(Your First Name), this month’s wellness challenge is ending soon.”  Please allow 72 hours after Offer Period for points to be credited to eligible registered Well members. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. Approximate Retail Value of 100 points: $1.00. Points redeemable in the Well Marketplace.  No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.

TÉRMINOS Y CONDICIONES

REQUISITOS PARA PARTICIPAR: La oferta está disponible para usuarios que (i) tengan dieciocho (18) años o más al comienzo del Período de la Oferta; y (ii) sean miembros registrados de la app de Well.

PERÍODO DE LA OFERTA: La Oferta “Bienestar en el lugar de trabajo” (la “Oferta”) está disponible a partir de las 9:00 a.m. ET del lunes 3 de junio de 2024 hasta las 11:59 p.m. ET del 5 de julio de 2024.

CANJE DE LA OFERTA: El patrocinador concederá 100 puntos de bonificación cada semana durante el Período de la Oferta a cada Usuario Elegible que complete un chat semanal sobre el “Bienestar en el lugar de trabajo”. El chat deberá contener la “palabra código” semanal. Se concederá un máximo de 400 puntos de bonificación.

CÓMO PARTICIPAR: El usuario deberá escribir en el chat la palabra o frase clave especificada en los correos electrónicos enviados con los asuntos siguientes en un plazo de 7 días tras haber recibido cada correo electrónico: “(Tu nombre), gana 400 puntos extra este mes”; “⏰Es hora de un descanso para tu bienestar, (tu nombre)”, “Te esperan tus puntos Well sobre el lugar de trabajo” y “(Tu nombre), el reto sobre el bienestar de este mes termina pronto”. Transcurridas 72 horas desde la finalización del Período de la Oferta, se abonarán los puntos a los usuarios de Well registrados que sean elegibles. Los puntos se pueden canjear en el Mercado de Well. No se sustituirán los puntos ni se canjearán por efectivo, a menos que lo determine el Patrocinador a su entera discreción. Valor minorista aproximado de 100 puntos: $1,00. Los puntos se pueden canjear en el Mercado de Well. EXENCIÓN/LIMITACIÓN DE RESPONSABILIDAD: Al participar en la Oferta, los participantes acuerdan aceptar y cumplir estos Términos y condiciones. Además, acuerdan que cualquier disputa relativa a cómo se lleve a cabo o se gestione esta Oferta o a la interpretación de estos Términos y condiciones se trasladará al Patrocinador, cuya decisión será vinculante y definitiva. Asimismo, al participar en esta Oferta, los participantes acuerdan eximir de responsabilidad al Patrocinador, así como a sus agentes y minoristas, afiliados, representantes de ventas, distribuidores y agencias promocionales u otras agencias proveedoras, sus respectivas sociedades matrices, afiliadas, filiales, agencias de servicio, contratistas independientes, y los directivos, administradores, empleados, agentes y representantes de cualquiera de las organizaciones anteriores (en conjunto, las “Partes eximidas”) de cualquier lesión, pérdida o daño a la persona, incluido el fallecimiento, o propiedad debido, en total o parcialmente, directa o indirectamente a la participación en esta Oferta o en cualquier actividad relacionada con la misma.

El Patrocinador no es responsable de ningún error tipográfico o de otro tipo en los impresos de la Oferta, la administración de la Oferta o errores relativos al anuncio de los requisitos de idoneidad o participación en la Oferta. A su entera y absoluta discreción, el Patrocinador se reserva el derecho a modificar, cancelar o suspender esta Oferta, o a modificar los Términos y condiciones en cualquier momento, sin aviso previo, en caso de que cualquier factor afecte a la administración, la seguridad o el desarrollo adecuado de esta Oferta, según se indica en estos Términos y condiciones.
