OFFICIAL RULES  

NO PURCHASE NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED BY LAW. BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE CHALLENGE, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT SO READ THEM CAREFULLY BEFORE ENTERING.  ALL FEDERAL, STATE, AND LOCAL LAWS AND REGULATIONS APPLY.

WELL APP:  The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis.  The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD:  The Complete your wellness visit Challenge (the “Offer”) is available beginning on January 1, 2026, at 8:00 a.m. Eastern Time (“ET”) and ends on September 30, 2026 at 11:59 p.m. ET (the “Offer Period”).  

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) Amentum Services, Inc. (“Amentum”) employees enrolled in medical benefits with Anthem HSA Gold and their medically-covered spouses; (iii) and a registered member of the Well application(“Eligible Person(s)”).

PARTICIPATION REQUIREMENTS: To participate, Eligible Persons must complete a preventive health visit during the Offer Period (the “Participation Requirements”). Preventive health visits may be fulfilled by visiting your primary care provider or OB/GYN for preventive care or other health needs.

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”):  Please allow two (2) weeks after Well’s receipt of external data files for the challenge to be marked complete.

Upon completion of the Challenge, Well shall notify Amentum that the Participation Requirements have been satisfied in order for Amentum to award a $200 HSA credit to the Eligible Person. 

Amentum will be the sole distributor of HSA credits. Please refer questions related to such distributions to the Amentum Benefits Service Center at 844-705-4099, available 8 a.m. to 8 p.m. Eastern Time, Monday through Friday.

Only one prize per person will be awarded. Prizes are not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES:  By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final. Additionally, it is expressly understood that utilization of the Well App requires ongoing acceptance of the associated terms and conditions. For more information, please review this page. By participating in this Offer, participant agrees to release and hold harmless Sponsor and its affiliates, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

CHANGES TO OFFICIAL RULES:  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper administration of this Offer as contemplated by these Official Rules.
