“USAA Premium Discount Challenge” Rules

TERMS & CONDITIONS

NO PURCHASE NECESSARY. Void where prohibited. All federal, state, and local laws and regulations apply.

SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to: (i) United Services Automobile Association (USAA) employees (collectively, “Eligible Users”).

OFFER PERIOD: The “USAA Premium Discount” Rules (the “Offer”) is available beginning 8:00 am October 1, 2024 and ends at 11:59 pm on August 29, 2025.

OFFER REDEMPTION: USAA will award rewards or premium discounts to every Eligible User who completes the participating actions of the Charlie, Bravo, and Alpha levels during the Offer Period as defined below.

Upon completion of the Charlie Level Challenge, Eligible Users will receive a choice of a BeWell@USAA-branded polo shirt, insulated tumbler, or lunch cooler. Allow 24 hours to receive an email confirmation of the Charlie Challenge completion; the email will include a link to an order form and a unique redemption code for order fulfillment.

Upon completion of the Bravo Level Challenge, Eligible Users will receive a 2026 premium discount at the following values:

Upon completion of the Alpha Level Challenge, Eligible Users will receive a 2026 premium discount at the following values:

You + Family	$1400 premium discount

TO PARTICIPATE: An Eligible User must first create an account with Well.
On mobile: Download the “Well Digital” app from the App Store or Google Play. Enter your USAA email address, date of birth, and mobile phone number to activate your account.
On the web: Visit go/BeWell  You will be automatically redirected via SSO to complete enrollment. Enter your USAA email address, date of birth, and mobile phone number.

To Complete the Charlie Level of the Premium Reduction Challenge
Complete the online health risk assessment on Well.

To Complete the Bravo Level of the Premium Reduction Challenge:
(1) Complete the Charlie Level Challenge, and
(2) Compete 12 Challenges with Well. Challenges can be found within “Rewards” on both Well mobile and web.

To Complete the Alpha Level of the Premium Reduction Challenge:
(1) Complete the Bravo Level Challenge, and
(2) Complete 24 total Challenges with Well (12 in addition to the 12 required for completion of the Bravo Level Challenge). Challenges can be found within “Rewards” on both Well mobile and web.
(3) Complete either a preventative care visit or a biometric screening.

Please allow reasonable and appropriate time periods for processing of rewards. Charlie Level awards will be distributed through Well’s partnership with its rewards vendor. As to premium incentives and cash equivalents, USAA will be the sole distributor of such rewards. Please refer questions related to such distributions to USAA’s Health & Welfare Benefits Center at 800-210-8722, Option 5. 

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.
Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
