SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304, Newton, MA 02464.

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period, (ii) a registered member of Well, and (iii) United Services Automobile Association (USAA) employees, or (iv) the spouse or domestic partner of such employees, provided that the spouse or domestic partner is also enrolled in the USAA Medical Program (collectively, “Eligible Users”).

OFFER PERIOD:  The “Fight the flu” Offer (the “Offer”) is available beginning 9:00 a.m. ET October 1, 2024 through 11:59 p.m. ET December 31, 2024.

OFFER REDEMPTION: Sponsor will award 125 bonus points to each Eligible User who meets the Participation Requirements.

TO PARTICIPATE: An Eligible User must receive a flu vaccine between August 1, 2024 and December 31, 2024. Completion of qualifying actions noted herein prior to the offer period will not invalidate eligibility. There are three ways an Eligible User can complete the challenge: (1) If you are enrolled in the USAA Medical Program and you’ve opted to share claims with Well, this will automatically complete when Well receives your claims data; (2) Complete the “Flu shot. Did you get it?”questionnaire, or (3) Let a Well Guide know you’ve completed your visit. Please allow two weeks after Well’s receipt of external claims data files or 72 hours after completion of the questionnaire or Well Guide attestation for points to be credited to registered Well user.  Points redeemable in the Well platform under Rewards > Redeem. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. 

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
