TO PARTICIPATE: An Eligible User must complete a preventative care visit between July 13, 2024 and August 29, 2025. Completion of qualifying actions noted herein prior to the offer period will not invalidate eligibility. Well exams, colon cancer screenings, and routine colonoscopies, mammograms, and Pap smears qualify. There are three ways an Eligible User can complete the challenge: (1) If you are enrolled in the USAA Medical Program and you’ve opted to share claims with Well, this will automatically complete when Well receives your claims data; (2) Complete the “So… when was your last annual checkup” questionnaire, or (3) Let a Well Guide know you’ve completed your visit by chatting through Ask Well. Please allow two weeks after Well’s receipt of external claims data files or 72 hours after completion of the questionnaire or Well Guide attestation for points to be credited to registered Well user.  Points redeemable in the Well platform under Rewards > Redeem. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.
