SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to: (i) Sam’s Club Members who have been granted access to the Well Application through their validated Sam’s Club membership, thereby becoming a “Well Member”; are (ii) 18 years of age or older at the time of entry; and are (iii) a legal resident of the United States who physically resides in the United States (“Eligible User”).

OFFER PERIOD: The “Welcome to Well” Offer (the “Offer”) begins on 8:00 am June 11, 2024 and ends at 11:59 pm on December 31, 2024 and is triggered the day an Eligible User creates a Well account and becomes a “Well Member” and continues for six additional days.

OFFER REDEMPTION: Sponsor will award 500 bonus points when the Well Member completes at least one Well Guide chat and any three (3) Journey actions during the Offer Period.

TO PARTICIPATE: Sign into the Well application and complete at least one Well Guide Chat and and three (3) journey actions during the Offer Period. Please allow 72 hours after Offer Period for points earned to be credited to Well Member Accounts. Approximate Retail Value of 500 points: $5.00. Maximum awarded 500 points (Approximate Retail Value of $5). Points can be redeemed for Sam’s Club savings for particular products Sam’s Club has identified as supporting a healthy lifestyle, and/or offers described in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
