SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to: (i) Regions Financial Corporation (Regions) employees who are enrolled in a Health Plan offered by Regions  and, and (ii) the spouse or domestic partner of such employees; provided that the spouse or domestic partner is also enrolled in the Health Plan, (collectively, “Eligible Users”).

OFFER PERIOD: The “1,000 Point Sign Up” Offer (the “Offer”) is available beginning 8:00 am July 15, 2024 and ends at 11:59 pm on December 31, 2026.

OFFER REDEMPTION: Sponsor will award 1,000 points to every Eligible User who downloads the Well Application and creates an account during the Offer Period.

TO PARTICIPATE: An Eligible User must download the Well Application, and create an account to become a Well Member. If you have an Apple phone, find the app here: Well Digital. If you have an Android phone, find the app here: Well Digital. Activate an account using your Regions email address.

Please allow 72 hours after account creation for points to be credited to a Well Member account. Approximate Retail Value of 1,000 points: $10.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.
Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
