OFFICIAL RULES  

NO PURCHASE NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED BY LAW. BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE CHALLENGE, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT SO READ THEM CAREFULLY BEFORE ENTERING.  ALL FEDERAL, STATE, AND LOCAL LAWS AND REGULATIONS APPLY.

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

WELL APP:  The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis.  The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD:  The Charlie Level (the “Offer”) is available beginning on September 2, 2025, at 8:00 a.m. Eastern Time (“ET”) and ends on August 31, 2026 at 11:59 p.m. ET (the “Offer Period”).  

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) a registered member of the Well application; and (iii) United Services Automobile Association (USAA) employees (collectively, “Eligible Users”).

PARTICIPATION REQUIREMENTS: To participate, eligible persons must log in to the Well App and complete all 6 questionnaires in the Health risk assessment (available through the Wellness tab in MyHealth) between August 30, 2025 and August 31, 2026 (the “Participation Period”).

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”): Please allow 72 hours after completion of all required actions for the challenge to be marked complete.  Upon completion of the Charlie Level Challenge, participants will receive a choice of a BeWell@USAA branded polo shirt, Stanley tumbler, or hat. Please allow 24 hours to receive an email confirmation of the Charlie Challenge completion; the email will include a link to an order form and a unique redemption code for order fulfillment. The ARV of the prize represents Sponsor’s good faith determination.   Only one prize per person will be awarded.  

All federal, state, and local taxes on prizes are the sole responsibility of participants.   Prizes are not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute prize(s) of the same ARV if advertised prizes become unavailable for any reason.  All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES:  By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final. Additionally, it is expressly understood that utilization of the Well App requires ongoing acceptance of the associated terms and conditions. For more information, please review this page. By participating in this Offer, participant agrees to release and hold harmless Sponsor and its affiliates, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

CHANGES TO OFFICIAL RULES.:  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper administration of this Offer as contemplated by these Official Rules.
