Premium reduction

OFFICIAL RULES  

NO PURCHASE NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED BY LAW. BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE CHALLENGE, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT SO READ THEM CAREFULLY BEFORE ENTERING.  ALL FEDERAL, STATE, AND LOCAL LAWS AND REGULATIONS APPLY.

WELL APP:  The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis.  The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD:  The Premium reduction challenge (the “Offer”) is available beginning on November 16, 2025, at 8:00 a.m. Eastern Time (“ET”) and ends on November 15, 2026 at 11:59 p.m. ET (the “Offer Period”).  

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; and (ii) Octapharma Plasma employees enrolled in a United Healthcare medical plan (“Eligible Users”).

PARTICIPATION REQUIREMENTS: To participate, Eligible Users must complete all five (5) questionnaires in the Health Risk Assessment (HRA) and complete a visit with a primary care provider or OB/GYN for preventive care or other health needs  during the Offer Period (the “Participation Requirements”). A link to the HRA may be found within the Premium reduction challenge by clicking “Complete your health risk assessment.” The preventive visit may be completed by entering the date within the Annual checkup action, linked within the Premium reduction challenge under “Visit a primary care provider or OB/GYN.”

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”):  Please allow 72 hours after completion of all required actions for points to be credited to registered Well user accounts.  Upon completion of the Participation Requirements, Eligible Users will receive a premium reduction of $500 for 2027. Octapharma Plasma will be the sole distributor of premium incentives and cash equivalents.

Only one prize per person will be awarded. All federal, state, and local taxes on prizes are the sole responsibility of participants.  Prizes are not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES:  By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final. Additionally, it is expressly understood that utilization of the Well App requires ongoing acceptance of the associated terms and conditions. For more information, please review this page. By participating in this Offer, participant agrees to release and hold harmless Sponsor and its affiliates, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

CHANGES TO OFFICIAL RULES.:  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper administration of this Offer as contemplated by these Official Rules.
