“MGM Borgata Wellness Incentive Challenge” Rules

TERMS & CONDITIONS

NO PURCHASE NECESSARY. Void where prohibited. All federal, state, and local laws and regulations apply.

SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to  (i) MGM Borgata employees enrolled in an MGM Anthem Health Plan, and (ii) the covered spouse or domestic partners aged 18 years or older of MGM Borgata employees enrolled in an MGM Anthem Health Plan (collectively, “Eligible Users”).

OFFER PERIOD: The “MGM Borgata Premium Discount” Rules (the “Offer”) is available beginning 8:00 am October 1, 2024 and ends at 11:59 pm on September 30, 2025.

OFFER REDEMPTION: MGM Borgata will award rewards or premium discounts to every Eligible User who completes the participating actions as defined below during the Offer Period. All Eligible Users can earn a wellness credit of $20 per person to be applied to MGM employee paycheck (a $520 annual credit per person for 2026).

TO PARTICIPATE: An Eligible User must:

(1) Create an account with Well. On mobile: Download the “Well Digital” app from the App Store or Google Play, or on the web, visit app.well.co. Employees must enter the email address MGM has on file for you, your date of birth, and your mobile phone number to activate your account. Invited spouses or domestic partners should use the address to which their invitation email was sent.

(2) Complete either a preventative care visit or three (3) coaching sessions with a Life Time Coach between October 1, 2024 and September 30, 2025.

Please allow reasonable and appropriate time periods for processing of rewards. As to wellness credits, MGM will be the sole distributor of such rewards. Please refer questions related to such distributions to the MGM HR Shared Services Department. To open a case, members can call (855) 464-6747 Monday-Friday, 5;00 a.m. – 5:30 p.m. PT, or access from MyMGM.com. 

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.
Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
