Wellness Credit

OFFICIAL RULES  

NO PURCHASE NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED BY LAW. BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE CHALLENGE, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT SO READ THEM CAREFULLY BEFORE ENTERING.  ALL FEDERAL, STATE, AND LOCAL LAWS AND REGULATIONS APPLY.

WELL APP:  The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis.  The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD:  The Wellness Credit challenge (the “Offer”) is available beginning on October 1, 2025, at 8:00 a.m. Eastern Time (“ET”) and ends on September 30, 2026 at 11:59 p.m. ET (the “Offer Period”).  

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) MGM Borgata employees enrolled in an MGM Anthem Health Plan; (iii) the covered spouse or domestic partners of MGM Borgata employees enrolled in an MGM Anthem Health Plan; and (ii) a registered member of the Well application (collectively, “Eligible Users”).

PARTICIPATION REQUIREMENTS: To participate, Eligible Users must complete a preventive health visit or complete 3 Lifetime Health Coaching sessions during the Offer Period (the “Participation Requirements”). Completion of the Participation Requirements will be captured through external data files. Please allow up to 2 weeks after completion of required actions for Well to receive data files and the challenge to be marked complete

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”):  Sponsor will report to MGM Borgata on behalf of each Eligible Member who completes the Participation Requirements outlined above. MGM Borgata will award rewards or premium discounts to every Eligible User who completes the Participation Requirements during the Offer Period in the form of a wellness credit of $20 per person to be applied to MGM employee paycheck (a $520 annual credit per person for 2027). All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES:  By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final. Additionally, it is expressly understood that utilization of the Well App requires ongoing acceptance of the associated terms and conditions. For more information, please review this page. By participating in this Offer, participant agrees to release and hold harmless Sponsor and its affiliates, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

CHANGES TO OFFICIAL RULES.:  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper administration of this Offer as contemplated by these Official Rules.
