SPONSOR: Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; and (ii) employees of JACK Entertainment, eligible for, but not yet enrolled, in Well.

OFFER PERIOD: The “JACK 1,000 Point Web Sign Up” Offer (the “Offer”) is available beginning 9:00 a.m. ET Monday, December 18, 2023 through 11:59 p.m. ET Friday, December 22, 2023.

OFFER REDEMPTION: Sponsor will award 1,000 points to every Eligible User who creates an account using Well’s web portal at https://app.well.co/ during the Offer Period. Maximum 1,000 bonus points.

TO PARTICIPATE: An Eligible User visits https://app.well.co/ and creates an account using the email address JACK has on file for the employee, their date of birth, and personal mobile phone number to become a Well Member. Please allow 72 hours after the Offer Period for points to be credited to a Well Member account. Approximate Retail Value of 1,000 points: $10.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) a registered member of the Well application, and (iii) employees of JACK Entertainment.

OFFER PERIOD: The “JACK Consumer Web Feedback” Offer (the “Offer”) is available beginning 9:00 a.m. ET Monday, December 18, 2023 through 5:00 p.m. ET Friday, December 22, 2023.

OFFER REDEMPTION: Sponsor will award 500 bonus points for one Well Guide chat that offers feedback after logging in and exploring Well’s consumer website at https://app.well.co during the Offer Period. Maximum 500 bonus points.

TO PARTICIPATE: During the Offer Period, log in to Well’s consumer web portal at https://app.well.co and explore the site. Then, chat with a Well Guide through https://app.well.co to provide feedback on your experience using the new consumer web portal. Please allow 72 hours after Offer Period for points to be credited to registered Well user. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. Approximate Retail Value of 500 points: $5.00; approximate value of 500 points: $5.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.
