“1,500-Point Sign Up” Offer

Rules

NO PURCHASE NECESSARY.  Void where prohibited.  All federal, state, and local laws and regulations apply.  

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The Offer is open to employees of J.A.C.K. Entertainment who are eligible for Well (collectively, “Eligible Users”).

OFFER PERIOD:  The “1,500-Point Sign Up” Offer (the “Offer”) is available beginning 8:00 am February 17, 2025 and ends at 11:59 pm on December 31, 2025.

OFFER REDEMPTION:  Sponsor will award 1,500 points to every Eligible User who downloads the Well Application and creates an account, or who creates an account on Well’s web app, during the Offer Period.  

TO PARTICIPATE: An Eligible User must download the Well Application (“Well Digital” from the App Store or Google Play) and create an account to become a Well Member, or create an account on Well’s web application at app.well.co. To create an account, Eligible User to use the email address JACK Entertainment has on file for the Eligible User.

Please allow two days after account creation for points to be credited to a Well Member account. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Rules and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Rules shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, individuals, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Rules.
