“JACK Entertainment Invite a Coworker Referral” Offer

TERMS & CONDITIONS

NO PURCHASE NECESSARY.  Void where prohibited.  All federal, state, and local laws and regulations apply.  

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The Offer is open to registered Well users employed by JACK Entertainment.

OFFER PERIOD:  The “JACK Entertainment Invite a Coworker” Offer (the “Offer”) is available beginning 9:00 am October 30, 2023 and ends at 11:59 pm on November 10, 2023.

OFFER REDEMPTION:  Sponsor will award 1,000 points to every eligible registered user who meets the Participation Requirements. Maximum 10,000 bonus points.

TO PARTICIPATE (“Participation Requirements”): Invite eligible coworkers, aged 18 years or older, to download the Well app and sign up by logging into the Well Application, tapping “You” at the bottom of the homescreen, then “Share Well” and selecting “Invite Coworkers”; enter a coworker’s email address or mobile phone number to email or text an invitation. After the referred coworker (i) registers with Well during the Offer Period and (ii) provides the Well referrer’s name and email address to a Well Guide via the app’s Ask Well chat function during the Offer Period, both the referrer and the referred new member will be awarded 1,000 points.  Points awarded following interaction with Well Guide. Points redeemable in the Well Marketplace. Approximate Retail Value of 1,000 points: $10.00. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
