SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to: JACK Entertainment employees who work at either the Cleveland or Thistledown JACK Entertainment site in the city of Cleveland, Ohio; and who are enrolled in a health plan (the “Health Plan”) offered by JACK (“Eligible Users”).

OFFER PERIOD: The “JACK Cleveland vs. JACK Thistledown Referral Challenge” Offer (the “Offer”) is available beginning 8:00 a.m. ET on April 17, 2023 and ending 11:59 p.m. ET April 30, 2023.

TO PARTICIPATE: Each Eligible User has been assigned to either the Cleveland or Thistledown JACK Entertainment site based on the employee’s work address (Cleveland or Thistledown). The regional team with the highest percentage increase in activation (defined by dividing the total number of eligible employees who create a Well account during the Offer Period by all eligible employees) will win the challenge. An Eligible User in the winning region, must download the Well Application, and create a Well account by the end of the Offer Period to receive the reward.

OFFER REDEMPTION: Sponsor will award 500 points to every Eligible User in the region that experienced the largest increase in activation percentage (as defined above). An Eligible User in the winning region, must download the Well Application, and create a Well account by the end of the Offer Period to receive the reward.
Please allow 72 hours after account creation for points to be credited to a Well Member account. Approximate Retail Value of 500 points: $5.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.

The Regional Team who’s Eligible Users have the highest average number of journey days completed during the Offer Period (defined by dividing the total number of journey days completed by the total number of activated users on the Regional Team) will win. An Eligible User on the winning Regional Team, must download the Well Application, and create an account to become a Well Member by the end of the offer period to receive the reward.

OFFER REDEMPTION: Sponsor will award 500 points to every Eligible User from the Regional Team who’s Eligible Users completed the highest number of average journey days during the Offer Period. An Eligible User on the winning Regional Team, must download the Well Application, and create an account to become a Well Member by the end of the offer period to receive the reward.
