“Holiday hero” Challenge

TERMS & CONDITIONS

NO PURCHASE NECESSARY.  Void where prohibited.  All federal, state, and local laws and regulations apply.  

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; and (ii) a registered member of the Well application.

OFFER PERIOD:  The “Holiday hero” Offer (the “Offer”) is available beginning 9:00 a.m. ET December 10, 2024 through 11:59 p.m. ET December 27, 2024.

OFFER REDEMPTION: Sponsor will award 500 bonus points to each Eligible User who meets the Participation Requirements. 

TO PARTICIPATE: An Eligible User must complete ten (10) healthy actions on the Well mobile app on any days during the Offer Period (any actions found on the Journeys tab of the mobile app, WG chats, MyHealth manual entries, and completing Questions of the Day); the days do not need to be continuous). Please allow 72 hours after completion of the actions for points to be credited to registered Well user.  Points redeemable in the Well platform under Rewards > Redeem or as credit toward employer benefit programs. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. 

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.

REQUISITOS PARA PARTICIPAR: La oferta está abierta a personas que (i) tengan dieciocho (18) años o más al inicio del Período de la Oferta y (ii) sean miembros registrados de la aplicación Well.

PERÍODO DE LA OFERTA: La Oferta “Héroe de las festividades” (la “Oferta”) está disponible a partir de las 9:00 a.m. ET del 10 de diciembre de 2024 hasta las 11:59 p.m. ET del 27 de diciembre de 2024.

CANJE DE LA OFERTA: El patrocinador otorgará 500 puntos de bonificación a cada usuario elegible que cumpla con los “Requisitos de participación”.

CÓMO PARTICIPAR: Un usuario elegible deberá completar diez (10) acción de salud en la aplicación móvil Well en cualquier día durante el Período de la Oferta (cualquier acción que se encuentre en la pestaña Recorridos de la aplicación móvil, chats con Guías de Well, entradas manuales en Salud y completar las Preguntas del día); no es necesario que los días sean continuos. En un plazo de 72 horas tras completar las acciones, se abonarán los puntos a los usuarios de Well registrados. Los puntos son canjeables en la plataforma Well en Recompensas > Canjear o como crédito para programas de beneficios de empleador. No se sustituirán los puntos ni se canjearán por efectivo, a menos que lo determine el Patrocinador a su entera discreción.

EXENCIÓN/LIMITACIÓN DE RESPONSABILIDAD: Al participar en la Oferta, los participantes acuerdan aceptar y cumplir estos Términos y condiciones. Además, acuerdan que cualquier disputa relativa a cómo se lleve a cabo o se gestione esta Oferta o a la interpretación de estos Términos y condiciones se trasladará al Patrocinador, cuya decisión será vinculante y definitiva. Asimismo, al participar en esta Oferta, los participantes acuerdan eximir de responsabilidad al Patrocinador, así como a sus agentes y minoristas, afiliados, representantes de ventas, distribuidores y agencias promocionales u otras agencias proveedoras, sus respectivas sociedades matrices, afiliadas, filiales, agencias de servicio, contratistas independientes, y los directivos, administradores, empleados, agentes y representantes de cualquiera de las organizaciones anteriores (en conjunto, las “Partes eximidas”) de cualquier lesión, pérdida o daño a la persona, incluido el fallecimiento, o propiedad debido, en total o parcialmente, directa o indirectamente a la participación en esta Oferta o en cualquier actividad relacionada con la misma.

El Patrocinador no es responsable de ningún error tipográfico o de otro tipo en los impresos de la Oferta, la administración de la Oferta o errores relativos al anuncio de los requisitos de idoneidad o participación en la Oferta. A su entera y absoluta discreción, el Patrocinador se reserva el derecho a modificar, cancelar o suspender esta Oferta, o a modificar los Términos y condiciones en cualquier momento, sin aviso previo, en caso de que cualquier factor afecte a la administración, la seguridad o el desarrollo adecuado de esta Oferta, según se indica en estos Términos y condiciones.
