ELIGIBILITY: The Offer is open to registered Well users who receive an email with the subject line “Get a pulse on heart health!”.

OFFER PERIOD: The “Bank of America Heart Health ” Offer (the “Offer”) is available beginning 9:00 am August 2, 2023 and ends at 11:59 pm on August 31, 2023.

OFFER REDEMPTION: Sponsor will award 500 points to every eligible registered user who meets the Participation Requirements for offer in its entirety. Maximum 500 bonus points.

TO PARTICIPATE – Blood Pressure Check In (“Participation Requirements – In-app”): Complete first blood pressure specific check in activity on Well app during offer period and receive 200 points. Complete at least seven (7) days of blood pressure specific check in activity on Well app during offer period and receive 200 points. Please allow 72 hours after Offer Period for points to be credited to registered Well user. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. Approximate Retail Value of 400 points: $4.00. Points redeemable in the Well Marketplace. TO PARTICIPATE – Well Guide Chat (“Participation Requirements-Chat”): Chat the code “Healthy Heart” once during Aug. 8-16 offer period and receive 100 points. Points awarded following interaction with Well Guide. Points redeemable in the Well Marketplace. Approximate Retail Value of 100 points: $1.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
