WELL APP:  The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis.  The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD:  The “Cruisin’ to 500 points” Offer (the “Offer”) is available beginning on June 3, 2025, at 8:00 a.m. Eastern Time (“ET”) and ends on June 27, 2025 at 11:59 p.m. ET (the “Offer Period”).  

ELIGIBILITY: The offer is open to individuals who are (i) a U.S. resident who physically resides in the 50 United States (includes DC); (ii) eighteen (18) years of age or older at the time of entry; and (iii) a registered member of the Well application (“Eligible User”)

PARTICIPATION REQUIREMENTS: To participate, Eligible Users must log in to the Well App and complete an Eligible Action on any 10 different days (these days do not have to be consecutive) during the Offer Period (“Participation Requirements”). “Eligible Actions” include, but are not limited to, any action with a point value associated, i.e., Question of the Day, Journey days, single day health activities, etc.; however, points from MyHealth streaks and Challenge completions are excluded. 

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”):  Sponsor will award 500 points to each person who has completed the Participation Requirements outlined above.  Please allow 72 hours after completion of all required actions for points to be credited to registered Well user accounts. Points redeemable in the Rewards section under the redeem tab within the Well App or toward a premium reduction program.  No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. The ARV of the prize represents Sponsor’s good faith determination.  The determination is final and binding and cannot be appealed.  Only one prize per person will be awarded.  

All federal, state, and local taxes on prizes are the sole responsibility of participants.  Redemption of points is subject to the terms and conditions of the Well Marketplace.  Prizes are not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute prize(s) of the same ARV if advertised prizes become unavailable for any reason.  All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES:  By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final. Additionally, it is expressly understood that utilization of the Well App requires ongoing acceptance of the associated terms and conditions. For more information, please review this page. By participating in this Offer, participant agrees to release and hold harmless Sponsor and its affiliates, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

CHANGES TO OFFICIAL RULES.:  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper administration of this Offer as contemplated by these Official Rules.

APP DE WELL: La “App de Well” se refiere a la aplicación o las aplicaciones de software exclusivas y propiedad del Patrocinador, bien sean aplicaciones móviles o bien sean como software a modo de servicio. La App de Well se puede descargar gratis desde la App Store de Apple (dispositivos iOS) o la Play Store de Google (dispositivos Android).

PERÍODO DE LA OFERTA: La Oferta “Navegando hasta los 500 puntos” (la “Oferta”) está disponible desde el 3 de junio de 2025, a las 8:00 a.m. Hora del Este (“ET”), hasta el 27 de junio de 2025, a las 11:59 p.m. ET (el “Período de la oferta”). 

ELEGIBILIDAD: La oferta está disponible para personas que (i) sean residentes de los EE. UU. y residan físicamente en alguno de los 50 Estados Unidos (incluido DC); (ii) tengan dieciocho (18) años o más en el momento de participar; y (iii) sean miembros registrados de la aplicación de Well (“Usuario elegible”).

REQUISITOS DE PARTICIPACIÓN: Para participar, los Usuarios elegibles deben iniciar sesión en la app de Well y completar una Acción elegible en 10 días diferentes (los cuales no tienen por qué ser consecutivos) durante el Período de la oferta (“Requisitos de participación”). Las “Acciones elegibles” incluyen, sin limitación, cualquier acción con un valor asociado en puntos, es decir, la Pregunta del día, los días de los Recorridos, las actividades de salud de un solo día, etc.; sin embargo, se excluyen los puntos de las rachas de Salud y de las finalizaciones de los Retos. 

CANJE DE LA OFERTA, PREMIO(S) Y VALOR MINORISTA APROXIMADO: El Patrocinador otorgará 500 puntos a cada persona que complete los Requisitos de participación descritos anteriormente. Espera 72 horas después de completar todas las acciones requeridas para que los puntos se acrediten a tu cuenta de usuario Well registrada. Los puntos se pueden canjear en la sección Recompensas de la pestaña canjear de la app de Well o en un programa de reducción de primas. No se sustituirán los puntos ni se canjearán por efectivo, a menos que lo determine el Patrocinador a su entera discreción. El valor minorista aproximado del premio representa la determinación de buena fe del Patrocinador. Esta determinación es definitiva y vinculante, y no se puede apelar. Solo se concederá un premio por persona. 

Todos los impuestos federales, estatales y locales sobre los premios son responsabilidad exclusiva de los participantes. Al participar en esta Oferta, el participante acepta liberar y eximir de toda responsabilidad al Patrocinador y a sus filiales, distribuidores y agencias promocionales y otros vendedores, a sus respectivas compañías matrices, filiales, subsidiarias, agencias de servicios, contratistas independientes, y a los directivos, directores, empleados, agentes y representantes de cualquiera de las organizaciones anteriores (colectivamente, las “Partes eximidas”) de cualquier lesión, pérdida o daño a personas, incluida la muerte, o a la propiedad debido total o parcialmente, directa o indirectamente, a la participación en esta Oferta o a la participación en cualquier actividad relacionada con la Oferta.

CAMBIOS EN LAS NORMAS OFICIALES.: El Patrocinador se reserva el derecho, a su entera y absoluta discreción, de modificar, cancelar o suspender esta Oferta, o de enmendar las Reglas oficiales en cualquier momento, sin previo aviso individual, en caso de que cualquier factor interfiera en la administración, la seguridad o la correcta gestión de esta Oferta, tal y como se contempla en estas Reglas oficiales.
