“2500 Point Sign Up” Offer

TERMS & CONDITIONS

SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to: (i) Bank of America employees who are enrolled in a health plan offered by Bank of America and administered by Anthem, Aetna, or UnitedHealthcare (the “Health Plan”), and (ii) the spouse or domestic partner of such employees; provided that the spouse or domestic partner is also enrolled in the Health Plan, (collectively, “Eligible Users”).

OFFER PERIOD: The “2,500 Point Join Well” Offer (the “Offer”) is available beginning 8:00 am March 8, 2022 and ends at 11:59 pm on December 31, 2026.

OFFER REDEMPTION: Sponsor will award 2,500 points to every Eligible User who downloads the Well Application and creates an account during the Offer Period.

TO PARTICIPATE: An Eligible User must download the Well Application, and create an account to become a Well Member.
Please allow 72 hours after account creation for points to be credited to a Well Member account. Approximate Retail Value of 2,500 points: $25.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.
Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.

ELIGIBILITY: The Offer is open to Bank of America employees who work at a Bank of America site and the spouse or domestic partner of such employees, in each case, (i) who are enrolled in a health plan offered by Bank of America and administered by Anthem, Aetna, or United Healthcare (the “Health Plan”), and (ii) who have downloaded the Well Application (collectively, “Eligible Users”).

OFFER PERIOD: The “Custom Gift Box” Offer (the “Offer”) is available beginning 8:00 am March 8, 2022 and ends at 11:59 pm on December 31, 2025 or until supplies run out.

OFFER REDEMPTION: Sponsor will award one premium gift box, while supplies last, to every Eligible User who downloads the Well Application, creates an account during the Offer Period (a “Well Member”) and completes the participation requirements below.

PARTICIPATION REQUIREMENTS: The Well Member needs to request a gift box by chatting “Balanced Plate” to a Well Guide using the Ask Well feature of the App. The Well Member will receive a gift box redemption code to order one of the gift box options from a partner website. The gift box options may vary. Gift box requests will be fulfilled by an external vendor, while supplies last. Well is not responsible for gift box fulfillment or contents. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

WELL APP: The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis. The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD: The Start Strong, Earn $5 challenge (the “Offer”) is available beginning on October 13, 2025, at 8:00 a.m. Eastern Time (“ET”) and ends on November 30, 2025 at 11:59 p.m. ET (the “Offer Period”).

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) a registered member of the Well application; and (iii) who see this challenge displayed in Rewards > Earn on the mobile app or Challenges on the web (“Eligible Member(s)”).

PARTICIPATION REQUIREMENTS: To participate, Eligible Members must log in to the Well App and complete at least six (6) actions within the first week of onboarding. Eligible actions include: Journey actions (any action available on the Journeys tab in the app), completing a question of the day, entering daily health status into Check-ins, chatting with a Well Guide about a health topic, engaging with an article on the mobile app, connecting a wearable device, completing the Health risk assessment, or engaging with a resource (available through the Well App) (the “Participation Requirements”).

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”): Sponsor will award 500 points to each member who has completed the Participation Requirements outlined above. Please allow 72 hours after completion of all required actions for points to be credited to registered Well member accounts. Approximate Retail Value of 500 points: $5.00. Points redeemable in the Rewards section under the redeem tab within the Well App. The ARV of the prize represents Sponsor’s good faith determination. The determination is final and binding and cannot be appealed. Only one prize per person will be awarded.

All federal, state, and local taxes on prizes are the sole responsibility of participants. Redemption of points is subject to the terms and conditions of the Well Marketplace. Prizes are not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute prize(s) of the same ARV if advertised prizes become unavailable for any reason. All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES: By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final.
