“Bank of America Healthy Lunch” Challenge

TERMS & CONDITIONS

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The offer is open to (i) Bank of America employees who are enrolled in a health plan offered by Bank of America and administered by Anthem, Aetna, or UnitedHealthcare (the “Health Plan”), and (ii) the spouse or domestic partner of such employees; provided that the spouse or domestic partner is also enrolled in the Health Plan, (collectively, “Eligible Users”).

OFFER PERIOD:  The “Bank of America Healthy Lunch” Offer (the “Offer”) is available beginning 9:00 a.m. ET Friday, March 1, 2024 through 9:00 p.m. ET March 31, 2024.

OFFER REDEMPTION: Sponsor will award 1,000 points to every Eligible User who sends a photo, or a description of, a healthy lunch to a Well Guide using the Ask Well feature within the Well application during the Offer Period. Maximum one redemption per Eligible User. 

TO PARTICIPATE: An Eligible User must download the “Well Digital” Application from either the App Store or Google Play, and create an account using their work email address to become a Well Member. Then, using the Ask Well feature, the Eligible User must chat a photo, or a description of, a healthy lunch to a Well Guide during the Offer Period.

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
