The “December Invited Dependent Bonus” Offer

SPONSOR: Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The Offer is open to the spouse or domestic partner of Bank of America employees who are enrolled in a health plan offered by Bank of America and administered by Anthem (the “Health Plan”), provided that the spouse or domestic partner is also enrolled in the Health Plan, who have received an invitation to create a Well account from the Well application,, (collectively, “Eligible Users”)

OFFER PERIOD: The “December Invited Dependent Sign Up Bonus” (the “Offer”) begins at 12:00 pm December 18, 2023 and ends at 11:59 pm on December 22, 2023.

OFFER REDEMPTION: Sponsor will award an additional 2,500 points each to the Eligible Users who complete sign up in the Well app within the Offer Period. Please allow 72 hours after Offer Period for points to be credited to registered Well user accounts. Approximate Retail Value of 2,500 points: $25.00. Points redeemable in the Well Marketplace. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

TO PARTICIPATE: The Eligible User must complete sign up for the Well app within the Offer Period.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
