ELIGIBILITY: The Offer is open exclusively to registered Well members who received this promotional offer via email, subject line “You’re eligible for a $50 gift card reward ✨” on February 25, 2025.

OFFER PERIOD:  The “3 Healthy Actions – Bonus” Offer (the “Offer”) is available beginning 9:00 am February 25, 2025 and ends at 11:59 pm on March 12, 2025.

OFFER REDEMPTION:  Sponsor will award 5,000 bonus points to each Eligible User who meets the Participation Requirements.  Please allow 72 hours after completion of 3 eligible actions for points to be credited to registered Well user accounts.  Approximate Retail Value of 5,000 points:  $50.00. Points redeemable in the Rewards section under the redeem tab within the Well application.  No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

TO PARTICIPATE: An Eligible User must log in to the Well application and complete at least 3 unique actions during the Offer Period. Eligible actions include: Journey actions (any action available on the Journeys tab in the app), completing a question of the day, or entering a daily Check-in activity.

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.

ELIGIBILITY: The Offer is open exclusively to registered Well members who received this promotional offer via email, subject line “🎁 Earn a healthy gift box and $50 in gift card rewards” between March 6, 2025 and March 12, 2025.

OFFER PERIOD: The “Bonus Gift box” Offer (the “Offer”) is available beginning 8:00 am March 6, 2025 and ends at 11:59 pm on March 12, 2025.

OFFER REDEMPTION: Sponsor will award one premium gift box to every Eligible User who completes the “3 Healthy Action – bonus” challenge and chats into a Well Guide “balanced plate” during the offer period.

TO PARTICIPATE: The Well Member must complete 3 healthy actions as outlined in the “3 Healthy Actions – Bonus” then they must chat in “balanced plate” using the AskWell feature feature of the App. The Well Member may select one of the gift box options and the Well Guide will provide the member with a link and promo code to redeem at an external vendor. The gift box options may vary. Gift box requests will be fulfilled by an external vendor. Well is not responsible for gift box fulfillment or contents. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.
