“$100 Healthier Eating” Challenge

TERMS & CONDITIONS

NO PURCHASE NECESSARY.  Void where prohibited.  All federal, state, and local laws and regulations apply.  

SPONSOR:  Well Dot Inc., 1254 Chestnut St, Suite 304. Newton, MA 02464.

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) a registered member of the Well application, and (iii) who receive an email with subject lines including the challenge name “$100 Healthier Eating Challenge.”

OFFER PERIOD:  The “$100 Healthier Eating” Offer (the “Offer”) is available beginning 9:00 a.m. ET Monday, November 20, 2023 through 8:00 p.m. ET Wednesday, January 3, 2024.

OFFER REDEMPTION: Sponsor will award a $100 Instacart Gift Card Code upon (1) enrollment in the challenge by chatting “Count me in for the $100 Healthier Eating Challenge” (or similar expression of interest) to a Well Guide and completing a two-question survey, (2) completion of chat participation requirements during all four weeks of the challenge, and (3) completion of a final three-question survey. Instacart Gift Code will be provided to qualifying members via Well Guide chat through the Well application. The Instacart gift card use will be limited to the following foods: fresh and frozen fruit, fresh and frozen vegetables, seafood, yogurt, nuts, seeds, beans, and nut butters.

TO PARTICIPATE: Enroll in the challenge before 8 p.m. ET December 1, 2023 by chatting “Count me in for the $100 Healthier Eating Challenge” (or similar expression of interest) to a Well Guide and completing a two-question survey through the Well application. On December 4, you will receive an email with the first chat prompt of the challenge; chat your response to the prompt by 9 p.m. ET December 8. On December 11, you will receive an email with the second chat prompt of the challenge; chat your response to the prompt by 8 p.m. ET December 15.  On December 18, you will receive an email with the third chat prompt of the challenge; chat your response to the prompt by 8 p.m. ET December 22. On December 27, you will receive an email with the fourth chat prompt of the challenge; chat your response to the prompt and answer a three-question survey by 8 p.m. ET January 3, 2024. Upon completion of all participation requirements, a Well Guide will chat an Instacart gift code with a value of $100 to you via the Well application. Please allow 72 hours after Offer Period for points to be credited to registered Well user. No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

RELEASE / LIMITATION OF LIABILITY:  By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final.  By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer.  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
